18. What is the best method of creating good NDS firmware dumps? Does anything need to be done to fix or check the file/s produced?

A lot. The firmware dump contains everything saved on the DS, from your favourite colour to your wifi settings.

19. What determines which DoM entry is the parent? Or is it just random?

General Europe version is parent, then the chain is US, JP etc. I think there's a document around that explains it...

20. What determines if an entry is labelled (Beta) rather than (Demo), and vice versa?
21. What determines the region of an entry?

21. What determines the region of an entry?
20) A simple two step process: Is it a demo? Is it a prototype of a released game? Prototypes are not demos in this classification.
21) The region is normally said in the header of a game, or in the licensing/copyright information.

Thanks

22. If a ROM is set to no region in the header and has no region info in the copyright info on the title screen, what region should the entry be set to? "World", "Unknown" or something based on the cart's distribution.
23. Is a DS BIOS properly dumpable if you reset the DS to factory settings via the menu, or only if the DS is never configured or switched on? If not, how is it properly dumped?

25. Is (Sample) just a outdated version of (Demo) (Kiosk) or (Demo), or is it for some other use? What should be used for E3 kiosk demos (i.e. kiosk demos which are also Betas)?

26. When should a dump source be marked as "Trusted Modification", and when shouldn't it?

Could you explain this, please?
My logic would be: First release is the parent, the following releases are the clones.

It might not have any logic behind it. It may just be for consistency.

I think Europe takes precedence because No-Intro can be considered European-based, yes?
I think it's not that different than how GameFAQs prioritizes American releases it is American-based (I think, at least assuming by the Wikipedia page that says it began on AOL, would imply it was founded in the US.)
